Difficulties

 The Task Force needs to gather some fundamental information on the provision of Motor Vehicle accident provision and issues for massage therapists in Alberta. Thus, the following survey has been developed.
  1. Download either AutoInsuranceSurvey.pdf or Survey.rtf
  2. print it out
  3. read it carefully
  4. answer it to the best of your abilities, and then
  5. mail it or fax it (mail and fax information on the survey forms) to the Task Force Chair by the
    deadline of September 1, 2006.

If you know of anyone that provides MVA massage therapy services, encourage them to submit a survey too. The more input we have from MVA massage therapy providers, the better our service and representation can be.
